Keep assumptions reviewable.
Record decisions and unresolved assumptions in a place stakeholders can access. Include why a decision was made and what would cause it to be reconsidered. This gives new participants context and helps the delivery team avoid rebuilding work around an undocumented change of direction.
Carry ownership into operation.
Confirm who manages changes and support after launch. Include the process for requesting updates and prioritizing issues. Project ownership and ongoing operational ownership may differ, so agree the transition before delivery ends rather than relying on the original project team indefinitely.
